Claims |
| To submit insurance claims, download and fill out the form below and fax or mail to the address provided on the form. Please note, all students are encouraged to use the SBI Pharmacy and the SBI Lab in Michael Hall on UB South Campus. For prescriptions, co-pays are usually less than at outside pharmacies and prescriptions are not subject to the off-campus $600.00 cap. For students covered by UB insurance, copays may be waived and prices for tests are mostly more affordable than at outside labs. |
