To enroll in the UB Student Medical Insurance,
please print and complete the appropriate form, follow all instructions, then mail or fax to the SBI Student Medical Insurance Office at UB.
Waiver:
The waiver forms for Fall 2009 will be available by
July 13th, 2009; no international waivers can be submitted before this time.
Please complete appropriate paper form then mail or fax to the SBI Student Medical Insurance Office at UB. Online waivers are not available for international or scholars.
OPT Participants
Enrollment is optional to OPT participants. No proof of insurance or waiver is required.
